It\u2019s a design problem.<\/p>\n<p>&nbsp;<\/p>\n<h3>The international workforce dimension: emergency medicine is highly dependent on non\u2011UK doctors<\/h3>\n<p>Part 1 of the report explicitly flags emergency medicine as having \u201cthe highest proportion of non\u2011UK PMQ doctors\u201d not only on the specialist register but also in SAS and LE roles working alongside specialists.<\/p>\n<p>This makes emergency medicine uniquely exposed to:<\/p>\n<ul>\n<li>changes in international inflow,<\/li>\n<li>changes in training eligibility rules,<\/li>\n<li>and shifts in perceived progression opportunities for non\u2011UK doctors.<\/li>\n<\/ul>\n<p>It also means policy rhetoric matters. The report\u2019s foreword warns that doctors are mobile and may choose other destinations if they perceive progression barriers or a hardening of support.<\/p>\n<p>Emergency medicine is exactly the sort of specialty where small shifts in retention can produce big operational holes.<\/p>\n<p>&nbsp;<\/p>\n<h3>The critical question: are LE roles in emergency medicine a pathway or a parking bay?<\/h3>\n<p>A ratio of 1.4 LE doctors per specialist could represent a robust model <em>if<\/em> LE doctors have:<\/p>\n<ul>\n<li>structured induction and supervision<\/li>\n<li>career development<\/li>\n<li>access to appraisal and revalidation support<\/li>\n<li>credible progression routes (including into training, SAS, or alternative senior pathways)<\/li>\n<\/ul>\n<p>But if LE roles function as short-term \u201crota glue,\u201d the model becomes:<\/p>\n<ul>\n<li>churn-heavy<\/li>\n<li>supervision-heavy<\/li>\n<li>and vulnerable to sudden recruitment shocks<\/li>\n<\/ul>\n<p>The report doesn\u2019t measure \u201cpathway quality.\u201d That\u2019s the missing piece leaders need to add.<\/p>\n<p>&nbsp;<\/p>\n<h3>What leaders should do: make workforce architecture explicit<\/h3>\n<p>If emergency medicine is being staffed by this mix, the plan must match the mix.<\/p>\n<p>Here are five concrete actions that align with the report\u2019s evidence:<\/p>\n<p>1) Convert \u201cpermanent reliance\u201d into permanent roles<\/p>\n<p>If LE doctors are a long-term structural component, keep fewer of them on fixed-term contracts by default. The data shows LE is overwhelmingly non-permanent, that\u2019s a churn engine.<\/p>\n<p>2) Strengthen SAS pathways in emergency medicine<\/p>\n<p>SAS contracts are more stable. If the service relies on senior non-consultant decision-makers, design that workforce on purpose.<\/p>\n<p>3) Reduce supervision overload on specialists<\/p>\n<p>High LE numbers mean high supervision needs.
